For those without insurance, we've got you covered at Lake Mills Family Dental! Explore our exclusive dental membership plans designed to make quality dental care more accessible and affordable for everyone.
Memberships are not insurance. All dental care must be provided by Lake Mills Family Dental, LLC. The program benefits are NOT transferrable to another dental practice. It is the member’s responsibility to schedule and keep all appointments offered as part of the program. Memberships are for one year. All benefits must be used during the one year period. Memberships begin on the day of purchase. Membership dues are payable in full at the time of enrollment and are non-refundable. Cash, check or credit card payments are accepted. Members will receive 15% off (13% if paid by credit card) any additional dental services not specifically listed as a member benefit. Payment is due on the date of service to receive the discount.
